  • Ankle Brachial Index (ABI) with doppler assessment for arterial perfusion
  • Monofilament testing to assess loss of protective sensation

Skilled Nursing Services may include but are not limited to:

  1. Teaching and Training in medication, diet, disease process, care procedure, and signs and       symptoms to report to physician
  2. Skilled Observation and Assessment of the patient to assess, observe, and intervene to avoid     complications
  3. Technical Skills wound care, administering injections to patients, catheter care, ostomy care,     and administering certain drugs allowable under Medicare guidelines. Home health care             focuses on teaching caregivers these skills if necessary
  4. Management and Evaluation to modify the plan of care to promote the patient’s successful         recovery and safety in the home
Ostomy Care, Skin care and containment, Continence Care, Foot Care, home health, WOUND & OSTOMY, wound clinics, Compression therapy,

Physical Therapy Service may include but is not limited to:


  1. Proper alignment/adjustment of CPM
  2. Instruct in TENS unit for pain management
  3. Educate in cold therapy and elevation for edema management
  4. Safety and fall prevention teaching
  5. Progressive strengthening and ROM HEP
  6. Balance re-training
  7. Gait training with appropriate assistive device
Ostomy Care, Skin care and containment, Continence Care, Foot Care, home health, WOUND & OSTOMY, wound clinics, Compression therapy,
Occupational Therapy Service may include but is not limited to:

1. Evaluate patients who have physical, developmental, social or emotional conditions
2. Provide patients with techniques that help them improve daily activities such as eating, bathing and dressing
3. Perform fall screens and evaluate older adults functioning in their homes and recommend specific home modifications
4. Modify tasks and the environment
Speech Therapy Service may include but is not limited to:

1. Initial screening for communication and swallowing disorders
2. Assess voice (hoarseness (dysphonia), poor vocal volume (hypophonia), abnormal (e.g. rough, breathy, strained) vocal quality
3. Evaluate cognitive aspects of communication (e.g., attention, memory, problem solving, executive functions).

Social Workers Assessment may include but is not limited to:
 
Provide counseling, evaluate a patient’s social and emotional factors, assist with long range planning, and help family members identify and utilize community resources. 

To qualify for services, treatment must be medically necessary and the patient limited in their mobility. The services are provided under the direction of a physician.


Elite Home Health Services accepts patients for treatment in the home on the basis of reasonable criteria and under the expectation that the patients’ medical, nursing, and social needs can be met adequately.

INSURANCES ACCEPTED


We accept many types of insurance plans but not all. We are able to accept Medicare, most Blue Cross Blue Shield, Cigna, Lifeprint/Optum, Humana, SCAN, Fresenius, and Medicare replacement plans. For Private Pay and Worker Compensation please contact our office.
